It means the technology we adopt today could be significantly better next year, which is different from, say, the transition from film to digital X-rays (that was mostly a one-and-done improvement in efficiency). With AI, we have to be prepared for continuous change and maybe continuous re-learning on the part of clinicians to understand the latest behavior of their AI tools. It’s less like installing a piece of equipment and more like hiring a new junior colleague who gets steadily smarter over time.

Scale of Impact Across Jobs: Many past automations in medicine ended up creating new kinds of work even as they eliminated old tasks. Take the move to digital health records – it was meant to automate note-keeping and make things efficient, but many doctors would argue it just changed the work (now they spend hours clicking dropdowns!). The introduction of PACS (Picture Archiving and Communication System) eliminated the job of film librarian and the hassle of physically fetching films, but it also increased the volume of images and enabled practices to read scans from anywhere, arguably increasing demand for radiologists. But in the long run, if screening volumes don’t explode to absorb that efficiency, it could mean fewer radiologists are needed for the same work. Not everyone is fully convinced that AI in healthcare is an unalloyed good. Healthy skepticism is warranted – after all, patient lives are at stake, and we’ve seen hype cycles in medicine before. Let’s examine some of the common counterarguments raised by skeptics (including some voiced by clinicians like Dr. Ainsley MacLean) and see how they hold up:


“AI is not a silver bullet – it can’t replace human expertise.”

No serious expert is suggesting we send women letters saying, “The AI cleared you, no human looked at your images at all.” Rather, the workflow is evolving into a human-machine partnership. In practice, what we’re likely to see (and what we advocate) is AI as a second pair of eyes, checking the checker, rather than replacing the checker outright. At least in the near to mid-term, Dr. MacLean’s stance holds: treat AI as an aide for radiologists, not a replacement​.


The moment we treat AI as infallible is the moment we risk patient safety. interestingly humans are infallible, and patient safety is put at stake when we forget that reality. So, skeptics who urge caution against blind automation are right – we need measured integration where clinicians retain ultimate responsibility and situational awareness. That said, as AI gets more capable, radiologists will need to find the right balance of trusting the AI’s input versus using their own judgment. Over-reliance (automation bias) can be as dangerous as under-utilization. The MASAI trial fortunately showed that radiologists can work with AI without a flood of false positives, but it will be an ongoing process to develop best practices for this teamwork.


“What about biases and blind spots in AI?”


Skeptic’s Viewpoint: AI models are trained on historical data, and if that data isn’t diverse or representative, the AI may perform worse on certain populations. We’ve seen examples in other domains where AI systems had racial or gender biases because of skewed training sets. In healthcare, this could translate to an algorithm that’s less accurate for, say, younger women, women of certain ethnic backgrounds, or those with atypical anatomy, if those were underrepresented in the training data. There’s also concern about AI handling outlier cases – for instance, extremely rare cancers might be missed if the model has virtually never seen them.


Our Analysis: Again, these are legitimate concerns. An AI that works great in Sweden (with a mostly Caucasian, 40-74-year-old screening population) might need retraining or validation before being used in, say, South Asia or in younger, high-risk patients. We should absolutely demand that AI tools undergo extensive testing across different subgroups and publish that data transparently. Regulators like the FDA are starting to require demographic breakdowns in AI validation. The MASAI trial was conducted in a pretty homogenous population, and the authors themselves acknowledge that generalizability to other healthcare systems or populations is a question​.


The good news is that AI can be improved – if biases are found, data can be augmented and models adjusted. But it requires awareness and commitment from AI developers and users to identify biases. Skeptics are right to flag this early. We need more diverse training datasets and perhaps multiple AI models tuned for different demographics if one-size-fits-all proves problematic. Healthcare equity should be a priority: we must ensure AI tools help all groups, not just the majority.


“Increased detection might not equal better outcomes.”


Skeptic’s Viewpoint: Finding 29% more cancers is great – or is it? The nuance here is that not every cancer found leads to saved lives. More detection is generally good, but quality matters over quantity. The MASAI trial was first and foremost a screening accuracy study – it looked at detection and false positives. The ultimate goal of screening is to reduce mortality. We won’t know for a few years if these earlier detections from AI translate into fewer deaths or less aggressive treatments. The researchers are following the participants for that exact reason. So, skeptics who point out “the jury is still out on outcomes” are correct​.


However, there is indirect evidence in favor of AI here: the fact that many of the additional cancers were invasive and node-negative suggests we are catching dangerous cancers earlier​. It stands to reason (and prior studies of screening vs. no screening support) that catching cancers at a smaller size before they’ve spread to lymph nodes will improve outcomes​. Moreover, MASAI did not boost the detection of ultra-low-grade lesions in huge numbers – if it had found a ton of borderline cases, the false positive rate would likely have spiked, which it didn’t. So, early signs are that AI is threading the needle of finding more significant cancers without too much overdiagnosis. Still, caution is warranted until long-term data is in. It’s good that even AI enthusiasts like Dr. Lång caution that we need to see interval cancer rates and cost-effectiveness before declaring total victory​.


In short: skeptics are right that we should measure success by patient outcomes, not just cancer counts. And so far, AI is checking the intermediate boxes; we hope and expect the downstream outcomes to follow, but it’s appropriate to wait for proof.


“Not every hospital or clinic can implement this easily.”


Skeptic’s Viewpoint: It’s one thing for a large academic trial or a well-funded health system to use AI; it’s another for smaller clinics, rural hospitals, or under-resourced regions to do so. Some detractors point out that many facilities might lack the IT infrastructure or capital to invest in AI systems right away​. There’s also a training curve – technologists and radiologists need to learn how to integrate AI into their workflow. If only wealthy hospitals adopt AI, we could widen healthcare disparities. Additionally, some healthcare providers might resist because of fear of change or concern about how it affects their roles.


Our Analysis: These are pragmatic concerns. Implementing AI at scale will require upfront investment – not just buying the software, but also ensuring data storage, cybersecurity, integration with radiology workstations, and training. For a small radiology practice, that can be daunting. However, we’ve seen rapid adoption of digital tech in the past once it becomes the standard of care. (For example, it was expensive to switch from film to digital mammography, but now essentially everyone has done so because the benefits justified it.)


One could argue that if AI truly halves the work, a small practice could handle more volume without hiring another radiologist – so there’s an ROI (return on investment) argument even for smaller providers. They might lease the AI service or pay per use, which could be cheaper than a salary. Still, to avoid disparity, policymakers and professional bodies might need to step in. We could imagine government grants or public-private partnerships to help resource-limited centers get access to proven AI tools, especially if they become part of standard screening recommendations.


As for staff training, radiologists coming out of training in the next few years will likely have exposure to AI tools during residency, making them more comfortable. Change management is always tricky, but the generation of radiologists now entering the field is quite tech-savvy and often eager to use AI (many have been hearing about it their whole training). The key is demonstrating that AI will make their job easier, not threaten it.


MASAI’s results can actually be reassuring it shows radiologists can maintain or improve quality and get relief from some repetitive work. That’s a win-win if framed correctly.
